Recognition of this anatomical variant is essential in establishing the stroke mechanism in bilateral paramedian thalamic infarcts. We describe the clinical presentation and neuroimaging characteristics in a patient with bilateral thalamic infarctions due to occlusion of the artery of Percheron. © 2010 Elsevier B.V.
