Some aspects of metabolic disturbance in post operative patients

Abstract

The tendency for the kidney to excessively retain salt and water in the early post operative period can lead to problems of circulatory overload if administration of salt and water is not carefully monitored. The interpretation of hyponatraemia (commonly due to water retention during this period) as sodium deficiency might lead to therapeutic error with grave consequences. The central role of magnesium in tissue repair and potassium repletion is emphasized. Chloride replacement is important in relation to acid base and potassium balance. Adequate calories is required to minimise endogenous protein catabolism. Calorie deficiency not only aggravates post operative catabolism but also impedes the anabolic process of healing. Certain specific sequelae of surgery involving the parathyroid glands are mentioned.
