Abstract: Different techniques for thumb reconstruction are described. The techniques are essentially based on careful assessment of the level, the type and the extent of the injury to the skeletal support and the surrounding soft tissues. The techniques that offer one stage reconstruction using local tissues to preserve the length and good sensory skin cover to the thumb with rapid rehabilitation and gratifying results should be preferred. However, in dealing with this type of injury, a full knowledge of the available methods for thumb reconstruction is essential. Each case is a problem of its own and the approach should be a flexible one. With the advancement of microvascular anastomosis, other techniques such as toe transfer or primary reimplantation of an amputated thumb should be regarded seriously as justified and established methods of restoring a functional thumb if the facilities and expertise are available.
