dc.description.abstractDiabetes is a chronic disease with increasing prevalence and detrimental complications when uncontrolled. The prevalence of diabetes and the financial burdens arising from the treatment of this disease and its complications continue to rise in Singapore. These trends have highlighted the need for new strategies to put a halt to the escalating diabetes burden. The collaborative care model, as a team-based approach, has been proposed to improve diabetes management where healthcare professionals share their expertise and resources to provide comprehensive and patient-centered care. However, evidence on its long-term effectiveness on self-care activities, quality of life, direct cost, and indirect cost is lacking. Therefore, the main aim of this thesis is to evaluate the sustainability of this care approach holistically in addition to the clinical outcomes, which included glycated hemoglobin (HbA1c), blood pressure, total cholesterol, and other lipid parameters, at various timepoints over a year of patient’s diabetes journey.
