Abstract: We describe the diagnosis and management of a patient with a progressively enlarging, non-healing ischiorectal wound. This patient was further evaluated with radiological investigations which showed the presence of a large left ischiorectal fossa mass. Histology confirmed this mass as an anal adenocarcinoma. Anal Adenocarcinoma is a rare condition that can arise from chronic inflammatory states. Treatment requires an abdomino-perineal resection with neoadjuvant therapy, and the goal of surgery is to achieve clear resection margins. � 2016, The Society for Surgery of the Alimentary Tract.
