dc.description.abstractDiabetes is a complex condition that can be managed effectively and efficiently through pharmacist-involved collaborative care. Hence, the main thrust of this thesis revolved around implementing and evaluating pharmacist-involved collaborative care in the primary care and community settings. The evaluation results showed that collaborative care can positively impact on clinical outcomes. Furthermore, patients in the collaborative care model reported an overall positive experience and were satisfied with the care received. In addition to improved humanistic outcomes of patients, the pharmacist-involved collaborative care appeared to be a cost-effective approach. In conclusion, this thesis demonstrated how pharmacist-involved collaborative care can effectively manage patients with diabetes. The findings of this thesis also paved the way for future studies that aim to improve the collaborative management of patients with diabetes and its sustainability in the long-term.
