The mainstay of treatment of metastatic trophoblastic disease is chemotherapy with meticulous follow-up using the most sensitive assays for hCG available. However, in selected cases, surgery has a definite place. It is important that the decision to operate should not be unduly prolonged, since a prolonged delay in the institution of such therapy carries a poorer prognosis. The combination of surgery and chemotherapy to appropriately selected patients gives a survival of more than 70%.
