Abstract: Systemic lupus erythematosus (SLE) is chronic autoimmune disease which spares no major organs and systems. Cognitive dysfunction, which affects executive function, learning and memory, is one of the commonest neuropsychiatric manifestations in patients with SLE. Subclinical cognitive dysfunction can occur in SLE patients without clinically overt neuropsychiatric symptoms as well. However, the neural basis and biological mechanism underlying cognitive dysfunction in patients without neuropsychiatric symptoms remain elusive. Fourteen new-onset SLE patients without neuropsychiatric manifestations and 14 matched healthy subjects were administered a modified Wisconsin Card Sorting Test (WCST) while the associated brain involvement was visualized by functional MRI (fMRI). SLE patients and healthy subjects showed comparable WCST performance on disease onset and after sufficient disease control, whereas SLE patients demonstrated increased brain activation to execute goal-directed tasks and altered neuronal activities of cortico-basal ganglia-thalamic circuit, hippocampus and amygdala, which are not associated with serum levels of anti-NR2 antibodies.
