Abstract: Persistence of epsilon-globin-positive-primitive fetal nucleated erythroblasts (PFNEs) beyond first-trimester in trisomy 18 fetuses has not been examined. Also, efficiency of fetal cell enrichment protocols for non-invasive prenatal diagnosis has not been tested in an appropriate in-vitro model system. Epsilon-globin-positive cells persisted up to term in trisomy 18 neonates. The efficiency of three-step enrichment-system using termination of pregnancy as an in-vitro model was 37%. An average of 3 and 10 epsilon-globin-positive-cells were enriched before and after invasive procedure in in-vivo model, respectively. A mathematical model was constructed to describe the relationship between pre- and post-termination number of fetal cells obtained in in-vivo model. This model had co-efficient of 0.35 and 90% prediction accuracy. A mathematical relationship for prediction of number of pre-termination PFNEs to be obtained through an enrichment-system was postulated. This thesis proves termination of pregnancy as a model system to study non-invasive prenatal diagnosis in the first-trimester using cell-based-strategy.
