Abstract: Hypercoagulation, increased platelet and platelet activity, enhanced fibrinolysis together with elevated t-PA antigen, uPA receptor, and PAI-1 antigen was seen in pre-operative ovarian cancer patients (n=36) compared to benign ovarian cyst cohorts (n=35). Further enhanced thrombin generation and fibrinolysis were seen in advanced stage of cancer, suggesting a state of an overly active disseminated intravascular coagulation. In tissue extracts, only u-PA antigen and PAI-1 levels were significantly increased in ovarian cancer (n=23) compared to benign ovarian cysts (=16). The study of relationship between haemostatic status and disease outcome showed significant associations of systemic shortened clotting times, reduced ATIII and plasminogen, and increased D-dimer levels with mortality of cancer patients. Of the 38 ovarian cancer patients studied, 16 patients died within 35 months from the diagnosis of disease and the overall survival rate for 36 months was 40.7%. It appeared that ATIII and D-dimer levels with known FIGO stage prognosis could be beneficial as systemic prognostic markers for survival outcome at least for the first 36 months of the disease.
