Abstract: Renal cell carcinoma is the most lethal of all urologic malignancies. Clinical upregulation of survivin, an inhibitor of apoptosis protein, is a molecular signature of unfavourable disease outcome and a diagnostic biomarker of tumour onset and recurrence. YM155, an inhibitor of survivin, was identified and an investigation was made to elucidate the possible mechanism(s) of action which accounts for its potent anti-proliferative activity on RCC. Its activity was found to be independent of VHL status and survivin levels and microarray analyses revealed that molecules associated with p53 and NF-kB signaling pathways were involved. In vitro combination of YM155 with sorafenib pointed to synergism and in vivo assessment of YM155 on patient-derived RCC xenograft in SCID mice showed a slowdown of the tumour xenograft growth. The positive impact of YM155 in the mitigation of cancer progression supports the view that it may be a potential molecular targeted agent in RCC.
